Change Hire Date to use jQuery DatePicker

I need to change the hire date to use jQuery datepicker in

[localhost]

Can anyone please kindly show me how to archive this? Thanks in advance!

I know. Just put the following code in <webapp name>/htdocs/protected/views/employee/create.php at

http://localhost:8888/logonapp/index.php/employee/create

oh got a problem here. After inserting the code, there is 1 more unknown textbox appear on the page. Any clue ?

Given the information provided, my best guess is that Magic Gnomes are causing the problem.
