I need a table to hold a range of decimals and their corresponding meaning e.g.

0.00-4.99 = poor
5.00-9.99 = average
10.00-14.99 = good
above 15.00 = excellent

what would be the best way to set up this table?

The best way would be to create a new table, "ranges": range_id, min, max, meaning.
